Instructions for documenting and reporting injuries occurring on UGA property and/or in the course of employment.


This information must be completed by a supervisor or departmental HR representative no later than the first working day following the injury or as soon as a supervisor is notified of any injury.

The information obtained is intended to document injuries either:

  •      Non work-related: non employees of UGA (including students, visitors, volunteers, guests)
    or employees outside the course of employment

  •      Work-related: employees of UGA (including faculty, staff, student employees, etc.)
    while in the course of employment (work related)
